Services Provided
- Lodging: in spacious private room
- Cleaning of the resident’s room/ Personal laundry
services, daily
- Nutritious meals: three meals per day, healthy snacks in
between meals
- Monitoring food intake and special diet
- Assistance feeding
- Assistance with taking prescribed medication: central
storing and distribution
- Assistance with activities of daily living: dressing,
bathing, grooming
- Continuous observation, care and supervision
- Incontinence management
- Bedside care for temporary illness
- Transportation arrangements for medical/dental
appointments
- A planned activity program, arrangement for utilization
of community resources
- Small exercise group/ Outside activities
- Supervision of resident schedules and activities
- Monitoring and appropriate reporting to families of
resident needs and condition
- 24 hour emergency response monitoring
Dementia & Hospice Care
Alzheimer’s and Dementia Care patient are accepted and Hospice
arrangements are made available to those in need
Walker or Wheelchair Dependent Residents
We also accept Walker or Wheelchair Dependent residents and a
small ratio of the following:
- Residents with Catheter
- Diabetes Type I and II (insulin dependents: if self
administered)
- Uses Oxygen
Other Services
- An individualized treatment plan after hospitalization
coordinated with the patient’s MD, Medical Social Worker, Physical
Therapists and RNs from Home Health Agencies, including but not limited to
special wound care, speech therapy etc.
- In-house doctors routine checkups monthly by patient’s MD
- Blood pressure, blood sugar monitoring
- Podiatrist scheduled for home visits bimonthly 4.5
Optional Services
- Professional hair and nail care services are offered
weekly in downtown Livermore. Transportation is provided by the facility. ·
